Community Outreach Program

The Community Outreach Program offers support to residents in their own homes in the greater Launceston area.

The support includes assistance with domestic tasks such as cleaning, cooking, washing and general unit care. Support may also include assistance with banking, budgeting, shopping and transport to important appointments.

Many clients recieve support in their units, or are met at their units to be assisted with shopping and banking in the community.

When - The Community Outreach Program operates during the year except for public holidays.

Fees - No fees apply.

This program is funded by the Tasmanian Government's Department of Health and Human Services (DHHS)

 

Individual and intensive support

Not everyone can fit neatly into a program. St Michaels always tries to meet the needs of the individual by being flexible with the type of support that is offered.

We currently provide intensive support to high-needs individuals. Depending on the needs and wishes of the individual this might be offered in their own homes, or in the community. We try to work with individuals within group settings if possible.

St Michaels is accredited with the DHHS as an Intensive Support Service (ISS) provider.

Self-directed support

With the support of the DHHS and commencing in 2007 St Michaels trialled a Tasmanian pilot program of Self-Determiing Shared Management Individual Support Package.

This program is on-going and offers administrative support and assistance for individuals who wish to 'self-manage' their own support funds.

During this time we became involved in the inaugural steering committee of InControl Australia, and have taken a lead role in encouraging debate and policy development in favour of self-managed funding models.

Fees - We apply a flat fee of 10% of the package value, subject to negotiation.
